Is this a scam "Remark Web Survey" with a check enclosed and verifying that a hugh amount of money has been won.

Most likely a scam, but I couldn't verify just from a quick search. If it sounds too good to be true it is. Don't give out any information you wouldn't give to a stranger on the streets. Identity theft is no fun.
