well what i would do is take him to the doctors maybe they can give you some tips.

i think you should just stick to one kind of milk! breast feeding can be quite stressing and the mother when she is stressed that can be passed to the baby i dont get it myself but some how it does.
it is easier to feed them on ready made up milk. when they have finished the milk always make sure you rub their back so that they burb it up, yes they may be sick but my neices and nephews were always sick but ones they get older they stop it. so pat and rub their back and they should be okay.

I agree with the previous answer , burping a baby and feeding slowly is very helpful to avoid vomiting from air bubbles, and keeping him upright a little time after feeding may be good too, however sometimes this does not help too much.
The first concern here will be if the baby is gaining weight as expected (see a growth chart using his birth weight) at the site below: http://www.kellymom.com/babyconcerns/growth/growthcharts.html
If he is not growing properly, a doctor visit is necessary ASAP. He may be allergic to formula (or lactose intolerant) or the mother is eating some foods which can irritate his stomach. Mother milk is the best and contains antibodies and other nutrients that formula does not have. When feeding both, there is a concern that the mother may not produce enough milk in the future and will need to switch to formula. Also, some babies do not want mother milk when given both and choose formula. His difficulties sleeping may be related to the stomach pain he experiences and his vomiting.
He may be better put on his side for sleeping than on his back because vomit may get in his breathing tube.
His pediatrician should be informed of his progress and concerns.

When my infant son 'spit' up a lot and cried all thru the night, no one, not even the doctor suspected he was allergic to the formula (back in those days it was Pet Milk and Karo syrup). He survived, but the parents almost didn't. Years later (15 to be exact) when I had a baby girl she did fine on breast milk, but then at a month, I had to go back to work and switched her to formula recommended by the pediatrician. Same, sad story all over again, but this time, it was projectile vomiting. The doctor told me to give it a while. I told him 'no', I wasn't giving it a while, if he didn't recommend a different formula, I would just change it myself. So he complied. There was no way I was going thru months of a baby crying all night. I don't know if this pertains to you, but milk/formula allergy has to be suspected. The other things which probably is not you case, is pyloric stenosis, where the opening to the stomach is closed.
